Sam Pan Take Away, a takeaway at 60 Damson Lane, Solihull, West Midlands was given the score after assessment on July 22, the Food Standards Agency's website shows.
It means that of Solihull's 113 takeaways with ratings, 72 (64%) have ratings of five and just one has a zero rating.
