Gregory's Fish & Chip Shop, a takeaway at 152-154 Green Lane, Castle Bromwich, Solihull, Birmingham was given the score after assessment on September 2, the Food Standards Agency's website shows.
It means that of Solihull's 115 takeaways with ratings, 72 (63%) have ratings of five and just one has a zero rating.
