Rice n P's, a takeaway at 8 Burtons Park Road, Smiths Wood, Birmingham was given the score after assessment on July 5, the Food Standards Agency's website shows.
It means that of Solihull's 111 takeaways with ratings, 70 (63%) have ratings of five and just one has a zero rating.
