Caspian Pizza, a takeaway at 453 Dudley Road, Birmingham was given the score after assessment on February 6, the Food Standards Agency's website shows.
It means that of Sandwell's 351 takeaways with ratings, 180 (51%) have ratings of five and just two have zero ratings.
