Spon Lane Fish Bar, a takeaway at 141 Spon Lane, West Bromwich was given the score after assessment on September 16, the Food Standards Agency's website shows.
It means that of Sandwell's 348 takeaways with ratings, 168 (48%) have ratings of five and just two have zero ratings.
