Neelams Kebab House also Trading as Neelams Pizza, a takeaway at 125-126 Poplar Road, Smethwick was given the score after assessment on July 27, the Food Standards Agency's website shows.
It means that of Sandwell's 348 takeaways with ratings, 167 (48%) have ratings of five and just two have zero ratings.
