Dixi Express Limited, a takeaway at 104b Oldbury Road, Smethwick was given the score after assessment on August 24, the Food Standards Agency's website shows.
It means that of Sandwell's 352 takeaways with ratings, 167 (47%) have ratings of five and just two have zero ratings.
