Kin, a takeaway at 20 Birmingham Road, Great Barr was given the score after assessment on August 19, the Food Standards Agency's website shows.
It means that of Sandwell's 352 takeaways with ratings, 167 (47%) have ratings of five and just two have zero ratings.
