Abbey Fish Bar, a takeaway at 14 Abbey Road, Smethwick was given the score after assessment on August 16, the Food Standards Agency's website shows.
It means that of Sandwell's 351 takeaways with ratings, 167 (48%) have ratings of five and just two have zero ratings.
