Newton Fish Bar, a takeaway at 18 Newton Road, Great Barr was given the score after assessment on May 18, the Food Standards Agency's website shows.
It means that of Sandwell's 361 takeaways with ratings, 169 (47%) have ratings of five and none have zero ratings.
