Tariq Sweet Centre, a takeaway at 152 Cape Hill, Smethwick was given the score after assessment on April 6, the Food Standards Agency's website shows.
It means that of Sandwell's 366 takeaways with ratings, 168 (46%) have ratings of five and just one has a zero rating.
