Le Le, a takeaway at 639 Bearwood Road, Smethwick was given the score after assessment on August 30, the Food Standards Agency's website shows.
It means that of Sandwell's 348 takeaways with ratings, 167 (48%) have ratings of five and just two have zero ratings.
