Cabin Chippy, a takeaway at 215 St Marks Road, Tipton was given the score after assessment on July 26, the Food Standards Agency's website shows.
It means that of Sandwell's 355 takeaways with ratings, 168 (47%) have ratings of five and just two have zero ratings.
