Chicago Pizza, a restaurant, cafe or canteen at 1760 Pershore Road, Bournville, Birmingham was given the score after assessment on September 23, the Food Standards Agency's website shows.
It means that of Birmingham's 1,734 restaurants, cafes and canteens with ratings, 912 (53%) have ratings of five and just 21 have zero ratings.
