Harbinger, a restaurant, cafe or canteen at 66 Bromsgrove Street, Birmingham was given the minimum score after assessment on July 7, the Food Standards Agency's website shows.
It means that of Birmingham's 1,679 restaurants, cafes and canteens with ratings, 988 (59%) have ratings of five and just 16 have zero ratings.
