Merkeb caffe, a restaurant, cafe or canteen at 30 Soho Hill, Birmingham was given the score after assessment on June 23, the Food Standards Agency's website shows.
It means that of Birmingham's 1,679 restaurants, cafes and canteens with ratings, 985 (59%) have ratings of five and just 18 have zero ratings.
